Admission Criteria & Process for Adult Day Services

Admission to Charlene's Angels Adult Day Center will be open to individuals meeting the admission guidelines without regard to sex, race, creed, or religious belief. 

Before placement, the Executive Director will meet with potential participants interested in attending Charlene's Angels Adult Day Center. The Director will evaluate each participant and determine if needs can be completed at the center. 

The ADS Level of Service Assessment Tool will be utilized to determine each participant's level of service using a point system. Charlene's Angels Adult Day Center will accept participants meeting Level 1 Service, which cannot exceed 11 points, and Level 2 Service, which cannot exceed 22 points.

To be considered for admission, the following guidelines are followed:

  • Participants must be 18 years or older.
  • Participants must be free from communicable diseases
  • Participants must undergo the ADS level of assessment to ensure the Center can meet the participant's needs.
  • Participants must be suitable to receive assistance from no more than one person.
  • Participant must be an ambulator (may use a cane or walker).
  • Participant must be continent of bowel and bladder. 

Charlene's Angels Adult Day center will accept participants who meet the criteria for the Basic Adult Day Service (Level 1 and Level 2) as follows: 

a. Monitor and/or supervise all Activities of Daily Living (ADL's are defined as dressing, bathing, grooming, eating, walking, and toileting) with hands-on assistance provided as needed

b. Comprehensive, therapeutic activities

c. Assure health assessment and intermittent monitoring of health status

d. Monitor medication

e. Ability to provide appropriate structure and supervision for those with mild and cognitive impairment

f. Staff to Participant ratio not to exceed 1:8

Before admission, the participant/responsible party will provide the Center with in-take documentation and general information, including medical and social history, emergency contact information, and other pertinent information. The participant will be required to complete and return the following items:

  • Participant Information Sheet
  • Request for Medical Information Forms & Current List of Medications
  • Request for Medication Monitoring, if applicable 
  • Release Form

Participants will provide to the Center copies of identification and power of attorney/guardianship documents and any advance directives.  

The participant/responsible party must complete a Consumer Contract. The Consumer Contract. The Consumer Contract will be signed by the participant/responsibility party and an administrative staff member. 

Management reserves the right to decline admission to any applicant for whom Charlene's Angels Adult Day Center determines, in its sole judgment, that it cannot meet his/her needs. 

 

Procedures for Acceptance of Services in the Event of a Wait List

 

If there are no current openings, the applicant will be placed on a waiting list based on the date of the inquiry. If there is a waitlist, the Executive Director will contact the next applicant according to the date of inquiry to set up an admission conference when an opening becomes available.
